THE SHOULDER IN THE ELITE ADOLESCENT TENNIS PLAYER: EXPLORATION OF STRUCTURAL AND FUNCTIONAL SPORTSPECIFIC ADAPTATIONS
The research findings presented in this PhD thesis highlight the structural changes/adaptations at shoulder level in the adolescent elite tennis player. In addition, a new field-test for eccentric force measurement of the external rotators is presented. The associations between tendinosis of the shoulder and aerobic and anaerobic energy systems were also explored.
In summary, in the dominant arm of the elite adolescent tennis player we have shown:
1) frequent infraspinatus tendinosis
2) alterations of the growth plate
3) associations between infraspinatus tendinosis and deficits in anaerobic power output. In addition, a new assessment of eccentric force output performed with an HHD has been presented and showed good to excellent reliability and validity.
TAKE HOME MESSAGE
The main finding and clinical “take-home message” from this thesis is that the shoulder of the elite adolescent tennis player needs to be assessed from a local and global perspective including the biomechanics of throwing, kinetic chain efficiency, functional and structural adaptations and development of the aerobic and anaerobic energy systems. In addition we must take into consideration the biological maturation process occurring parallel to the high-performance training and therefore encourage players to achieve a “train to be able to train” level as a first step in their professional career.
